what is the treatment of choice for Torsade de pointes?

this is what i know also, but there is indicaiton for : B blocker, cardioversion , and stopping the offending drug like pheonthiazine, TCA, quinidine, .. or other antiarrythmics.

so hypomagnesemia and treating with Mg is one modality of Rx. and not everything....

any one know ??

Therapy for Sustained TDPis immediate direct current
(DC) cardioversion ;; reversal or treatment of
other underlying condition should be done in a patient
of TDP or with QTc is > than 500msec .

MgSo4 IV bolus for treatment and prevention of drug
induced TDP.
Elimination of long-short triggering sitautions and shortening
of QT interval can be achieved by increasing heart rate by
isoproterenol infusion or temporary transvenous pacing.

true i can be used ,BUT FOR CONGENITAL LONG QT CASES
.. Patients with congenital long QT syndromes are thought to have an abnormality of sympathetic balance or tone and are treated with beta-blockers.
If the patient breaks through this therapy and enters the ED in torsade, a short-acting beta-blocker, such as esmolol, can be tried.
