It is ok to let the program you are most intereted to know that you are ranking them no.1. But from what I have learnt, it doesn't have any effect on moving you high on their list. Actually the recruiting persons for residency make point in not being biased by the personal communications. So I think it's better you don't expect the letter to have your chance being increased. But as I said doesn't hurt to let your program of choice know your interest.
