I think it is a general rule that if you have any attempt in any of the steps and require a visa , it's very much done for you ..... I havent seen any one getting IV's with attempts who require visa ...

I know one of the my friends who has better step 1 and ck scores than me (230's), but unfortunately failed on CS , and got no interview..... I think this is very harsh and everyone deserves a second chance , but unfortunately this is the situation.

and in general FM is said to be reserved for those who dont require visa's whether you have an attempt or not ... I think peds or may be IM could be a better option for you next year as SOAP is again almost reserved for those not requiring visa ...
