Salam,

I never did any externship or hands on experience and as I know hands on experience or LOR that's coming by money and through some institute doesn't have any value for program director. You can google about it and see it. Try to find observership by email and you can give 1-2 LOR from IRAN. One of my LOR was from IRAN from an unknown internist.
