please explain why not a .. Thank you...

86. A 16-year-old boy is admitted to the emergency department because of a knife wound to the left
side of his chest. An x-ray of the chest shows an air-fluid level in the left side of the chest, partial
collapse of the left lung, and elevation of the stomach bubble. The mediastinum is in the midline. Which of the following is the most likely diagnosis?

(A) Hemopneumothorax, not under tension
(B) Hemothorax, not under tension
© Pneumothorax, not under tension
(D) Tension hemopneumothorax
(E) Tension hemothorax
(F) Tension pneumothorax

Mediastinum is in the midline indicating that trachea is not deviated which excludes tension

A...hemopneumothorax because there is air-fluid levels, not under pressure because mediastinum is central?

Mediastinum is in the midline due to two equal and opposite forces.

1-collapse lung pulls mediastinum toward the affected side
2- tension pneumothorax pushes mediastinum toward the unaffected side

So these two equal and opposite forces keep the mediastinum in midline.
