For rank order lists should I rank the best program as no. 1 or no. 1 should be where my interview went best? Please advice.

The rank is for you. You rank programs in your order of preference. The match benefits you, you can't lose a program because you ranked them low and they ranked you high. You match in the first program that ranked you within available positions.
