Perhaps it depends on the doctor that you are working with, but I see it as a professional relationship tantamount to asking your boss to write you a reference letter for a job - I mean that is basically what it is. True, the school/you are paying the doctor for the education/experience but the actual process of applying for residency is a job application. I have given my bosses from other jobs gifts/thank you cards at the end of my time or for a letter so I don't see it as an ethical problem.
There are limits to how much to spend though, I agree on that part. I don't have a job yet, so I am thinking Secret Santa budget for all my LOR writers!
