An otherwise asymptomatic 36-year-old woman reports "swelling up" that has worsened over the past 4 weeks. She sometimes has one or two glasses of wine with dinner but does not smoke cigarettes. Her blood pressure is 160/95 mm Hg. Examination shows ascites and marked ankle edema. A few crackles are heard at the lung bases that quickly clear with deep breathing. Serum studies show:

Urea nitrogen 30 mg/dL
Creatinine 2.8 mg/dL
Albumin 2.0 g/dL

Urinalysis shows 3+ albumin and no cells. Which of the following is the most likely cause of the ascites and edema?

A. Acute tubular necrosis
B. Cirrhosis of the liver
C. Congestive heart failure
D. Glomerular renal disease
E. Interstitial renal disease
