2 month infant brought to ER by paramedics after his mother found him not breathing in his crib. he is apneic and cyanotic, no pulses are palable but he is warm. after manually stablizing his neck and positioning the airwa, which of the following is the most appropraite next step in managnment
a positive pressure venitlation and o2
b - two chest trsuts followed by chest compressings
c-establishing vascular access
d administer epi
e electrical defibrillation

answer is apparently A, can someone explain the pathology/treatment here?

because it is pulsless electrical activity ( baby is warm). You need to treat the cause which is apnea

airway: establish access

breathing: PPV!!

circulation: now work on the pulse.

in pt without pulse ABC is not the first step. CPR is the first step. Nowadays they don't even recommend to do breathing but chest compression only. In this case you will give O2 and intubate cause even that no pulse baby is warm, so you will eliminate the cause ( apnea...which is by the way the MC cause of pulses electrical activity)
