& yes u are correct.

When PTH levels are up in 1ry hyperparathyroidism*, hyperCamia -> results from ↑mobilization of Ca from bone and enhanced intestinal Ca absorption. Now as you mention as ↑ osteoclast activity would be prominent in cortical bone/subperiosteal & endosteal surface but medullary part of bone spare when you look at it u see those railroad tracks( dissecting osteitis) as bone PAIN u see most pat and can be evidence x-ray & as consequently => u see plasma ALP MOST time norm OR slightly up apt*.
