I worked with an internal medicine physician sometime ago and at the time she liked me, she was never mean to me, she was nice to me. So now I asked her for letter of recommendation and she said she would write it, still hasnt written it and ignores my messages, heard her call me crazy as I walked pass her, and she looks at me with anger in the clinic, its so weird. Idk what pissed her off? Gossip? I only asked her once for lor. Should I even use her letter assuming she uploads it.

Nope don't use her letter.
If she called you crazy that is a red flag.
And also your description of your experience with her was of " she was never mean to me", " she was so nice to me".
Nothing in the post suggests things like " She thought I was exceptional", " She promised to push my residency application"
Stay clear of her letter...so many red flags

dont use her letter.... it would be better
