I doubt you can find a paid externship at an ACGME institution.

You can try private clinics.

If you are looking for paid externship rotation then try to gain rotation through Medclerkships where you will be gaining strong inpatient hospital experience and will be getting Strong LORs. Here is the link which is free to submit the application http://.com/%20apply/

Medclerkships cheated lot of people. Try at your local hospitals or doctor offices.
