i think it should be P24.....that is the one that is usually detected by the p24 assay.....though even the hiv rna can be detected by PCR very early in the infection

I think it is P24. One of my friend who had a needle stick injury was asked to undergo a test to see if p24 antigeng is positive or not.

During first month of infection, Ab tests are usually negative. So serological tests cannot be used for diagnosis.
But HIV can be detected in this period by either viral culture, p24 Ag test or PCR assay. 10-20days after infection...increase in HIV RNA seen with PCR by 30days an increase in p24 can be seen in patients whose Ab tests were negative... In clinical practice...viral culture is done in only few medical centers. p24 Ag test is tested with Western blot which is the definitive test for diagnosis. I think u guys are right in terms of PCR as this the most sensitive and specific test and can detect HIV DNA within infected cells. Some individuals who have no detectable p24 will be picked up by this test. But this is not routinely done. p.s This is based on Levinson micro book info...
