24 year old male with three day history of testicular swelling progressive, no nausea vomitting, mild fever, dull pain, no radiation. Pain does not diminish on lifting the testis up but aggravates from 6/10 to 7/10.No risk factors for STD.
what s your diagnosis
a. torsion
b. epididymitis

yes this is torsion
fever can happened with any ischemia in the body so its not specific.all the q and especialy that last line tell u thiis is not std so from where infection come.

u people must be kidding or might not have seen torsion in real life its really painful and in three days it must have necrosed so q might have some other obvious clue
my bet is for b but its not in option but it sounds like mild testicular infection

This is epididmitis because 3 days h/o swelling,accompany mild to moderate.Phehns sign is usually but not universally present.
Testicular tortion is characterized by sudden develoment of moderate to severe pain.High riding testis can be palpated on affected side.

I agree with faraz, remember TORSION is presented always as acute condition, RULE OF THUMB.
