BST is "basic surgical training" (the first step in surgical training in the UK) and HST is "higher surgical training" - the specialization process in the UK that takes 5-6 years of surgical practise where the trainee is called " specialist registrar"
Currently, in many surgical specialities, there is a delay in going from the basic to higher grade. Also, at the end of the BST training (2-3 years where lots of surgical specialities are covered), there is the membership exam for one of the surgical colleges (England, Edinburgh, Gasgow or Ireland). The trainee receives the title MRCS (or equivalent depending on the college that the exam was taken). After this is taken, the trainee traditionally loses the title of "Dr." and becomes "Mr." ot "Miss", that implies surgical status. Its as confusing as the rules of English Cricket, but I hope that helps !!
