ok glucocortiocids is part of the stress hormones (epi/cortisol/glucagon). They have the opposite effctse of Insulin. When blood gucose is low they kick in and promote glusose release into blood.
Take Somogy efftect for example-- Pt takes insulin before bedtime (most likley intermediate or NPH type insulin as its action peak 7-10 hrs after intake) and wakes up in the middle of 3 am being hypoglycemic and at 7 am hyperglycemic. Why is he hypo at 3 am. B/c he probably got to much of a dose of NPH which made him hypog and this triggered the release of stress hormone which caused incr of glu in the blood and by 7 am Pt became hypergly. So in this case we have to decr the bedtime dosage of insulin to avoid Pt becoming hyop at 3 am. Another example is w/ GH= acromegaly. Myabe you can read that and tell me the effcts of Acromegaly and blood gucose levels in theses Pts |
