always ask few lines.
Start by asking that I would like to ask u few questions about ur sexual history , is that ok with u? Then ask regarding R u currently sexually active, with how many partners, what mode of contraception do u use, did u ever had STD?

for pediatric cases, you do not need to ask sex hx.
