first of all preliminary could be non-designated or designated prelim which means one year of general surgery then to ortho or whatsoever. Most IMG will apply to prelim because categorical general surgery is very competitive and after doing one year of prelim, you will apply for categorical general surgey and that way you will have a very good chance of going into categorical side. You have to repeat PGY1 for that matter and some people will do 2 years of prelim just to increase their chance into categorical gen surg. THERE'S IS NO WAY THAT AN INTERNIST TO BE PAID HIGHER THAN A SURGEON. After gen surg you could always do minimal surg or Plastic surg and by that time you are at par with AMG cause you did 5 years of American training. Pls look up salaries in any web and surgeon are always about 50k higher than IM and I'm not saying this casue I got prematch but I just love surgery and it is the ultimate medicine for sure
