guys..

this chap is pale...diaphoretic....tachycardic

there is no neurology

he is an old man
there is calcification anterior to vertebral bodies ...suggestive of aneurysm

u dont get such amount of pain in lumbar stenosis

herniated nucleus pulposus...should have some positive examination finding

this is plain case of ruptured aortic aneurysm...this is what i believe

BP is getting Low and Pt has Tachyc as a compesatory mechanism to BP Loss
