male, around 50y/o, has developed gynecomastia and and small firm testes
low TSH, LOW T3 T4, what is the cause

heyArchie, do u remember choices.
We are so trained having choices its little bit difficult to think without it.

It may be due to pituitary tumor, e.g. prolactinoma. Prolactin causes gynecomastia and damage of pituitary tissue causes low TSH, thus low T3 and T4.

I remember this q too, I think I answered prolactinoma, cose he had a double vision

prolactinoma --> Inhibition of Pit axis --> decr GnRH --> Small Testes
