how many of you sent this ' ERAS Document Submission Form' with your documents.
I just realized this form must be attached with all documents.

I am asking this because I did not send this form, still waiting for the documents to be scanned, sent documents only 2 weeks ago.
Did anyone not send the form, and have the documents scanned on the system. please..

NEES.. your docs are going to take forever to get scanned.. the DSF has a barcode to make things easier for the people at ERAS.. that was really stupid not to send the DSF. good luck
