for thyroid masses. first step is tsh, if it is high measure t4 and also draw antithyroids abs since hashimoto can present like that. however if nodule is >1.5 cm do FNAB high probability of malignancy,. if tsh is low do radioactive iodine uptake, if hot nodule is not malignant if cold do FNAB..if tsh comes back normal do FNAB.....source UW.
