Suspected myelomeningocele patient.
what is the next step ( first step) to Dx? and what is the test to Diagnostic confirm of it?.

suspected neonatal with myelomeningocele.
what is the initial test (first test) to diagnose?. and what is the test to confirm?.( i meant, the neonate defect the lumbosacral region. how to different b/w meningocele and myelomeningocele by the test).

X.ray or MRI or CT?

x ray will be the initial test to know the primary vertebral defect.
CT/MRI will be confirmatory and will show the extent of the lesion.

The initial test is antenatal maternal serum AFP which is rised and if the dates are right then amniotic fluid acetyl cholinesterase and AFP are used to diagnose neural tube defects.

If it's not diagnosed antenatally( very rare not to diagnose meningoceleor myelomenigocele but possible with occulta), then it's pretty obvious clinically. To differentiate between these two precisely, MRI is the best test.( For occulta, ultrasound is the initial test followed by MRI)

If the choices are limited as given by you, X ray followed by MRI
