A 48-year-old man comes to the office complaining of "chest pain." He states that he has been having episodes of "vague chest pain" for the past few years when he is at home with his wife. The pain does not occur when he is sleeping or when he is away from the house. He is a traveling salesman and he says that he enjoys his work and likes "being on the road."
