it is true simple but very important:

In general we initial do stabilization with heart rate and blood pressure control with Beta blockers such IV propanolol followed by IV nitroprusside.
Type A Ascending Dissection treatment surgical repair
Type B Descending (distal to the left subclavian artery) Medical approach preferred, unless there is intractable pain or progressive dissection.

debakey type one needs surgery because it is involving coronary arteries and patient will be in cardiogenic shock which will be further dangerous when we decrese blood pressure,

debakey was the first surgeon who did the vascular graft -- nobody know's his wife name who made or sew the vascular graft. but hats off to both of them.
