it depends what is possbile cause of fatigue. You should figure it out from history, then decide which should be focused on.

some suggestions:

Occult malignancy (most likely pancreatic or liver cancer)--focused on abd, skin.
Hypothyroidism: thyroid and some general PE.
Depression: general PE (including thyroid),sometimes need MMSE.
HIV infection: mouth,skin, and abd,etc.
hepatitis--abd, skin
chronic fatigue syndrome--general PE
