do i need to say my DD to the patient right after history and after physical exam; or just after physical exam;

He may ask after hx but resist by politely saying that at that point you still need to examine and likely run few tests, but after exam please keep it as vague as possible and use simple terms to describe to him at most 2 or 3 didd dx
