According to ECFMG you should use the format for LOR given by ECFMG..

That includes:
Name
AAMC ID

Addressing "Dear Program Director"

It's better to have recent LOR (Usually not older than 1 year)

But you still can use the LOR as long as it got you name and the writer's official letterhead.

AAMC id is for them to identify easily. It will be matched to your name and aamc id.

If you send lor without aamc id, it is a little difficult for them to match your name. They are worry that they will put in other slot with the same name. That's why they asked.

Last year I sent one without aamc id but it worked though. I got interviews and I saw that lor during iv.

Don't forget to use cover letter for lor, which has all your information.

I would also write your aamc id in pencil on the back in addition just in case they misplace it.
