Thanks for the article, it looks familiar is it from Iverson guide on residency and interview?

To add to make the LOR complete: ERAS requirement
1. official letterhead, signed in blue ink, if possible have the institutes seal.
2. Address to "Dear Program director".
3. have your name or AAMC ID in the subject matter or in the body.
4. Waived or not.

But if you give the writer the LOR cover sheet, disregard what is written above as its included there.
