I heard that before you start residency in any Pennsylvania residency program, the state has to contact directly with your home country to verify everything such as transcript and diploma. Is that right? Is it before or after visa processing? Any info? Thanks!

I think that's the requirement of the state medical boards. In most states you have to have a temporary state medical license (or so-called institution permit) to start residency. Most state medical boards review your original diploma and credentials and will contact the original country in cases that the documents are not complete. But I don't think they do that for every case. I don't know specifically about PA though.

For J1, I am not sure if state license is required for obtaining the visa, but as I said is required for starting residency in most states.
