Distinguish between joint pain and muscle pain or other nonspecific pain. Check range of motion and strength of larger joints, and if joints are not involved and motor strength is symmetric and 4+ to 5/5, consider fibromyalgia in any patient with generalized body aches without true weakness. Palpate the 18 "tender points". 11 or more points of 18 positive makes fibromyalgia likely. Other symptoms of conditions that support fibromyalgia are migraine headaches, sleep disturbances, irritable bowel syndrome, vague back pain, and depression. No lab tests are diagnostic, but normal labs - like no anemia, normal sed rate or CRP, normal CPK level, normal TSH, RF, ANA, etc - that rule out other serious conditions all support fibromyalgia. Best of luck to you from cmcases of competentmed.com
