how much you write on the scrap paper..so that it is enough to morize the hx.

It depends on your memory. The more you can remember, the less you write. It is hard to do it without loosing eye contact, therefore you do it as you paraphraze the pt's words (gaining an extra point for that). I would not trust my memory in such a stressful situation and with the number of patients (12), and write as much as I can w/o compromising a contact.
