5 Year cut off is waived off by most programs if the person is practice of clinical medicine and if he can explain the gap
He has decent scores and he would be a strong candidate for IM
It wont be wise to apply in any other specialty than IM

FM and Psych Can be Options but we would advise in your particular case to focus on IM
Why we are saying that FM and psych PDs like to see genuine interest (through research, observership, externship) in that specialty
For applying into multiple specialties your bf has to present a profile which can be justified in multiple specialties which would increase chances slightly for FM and Psych but would decrease chances of IM to a greater extent
So better be Master of ONE than being JACK of many
