24 yrs old girl with diarrhea - psych80

a 24 yrs old girl comes to emergency with compplain of profuse watery diarrhea with yellow flecks.. she has h/o URI resolved with medication..

what routes of administration are employed for flagyl and vancomycin in this patient for her diarrhea..?

Oral for Both.

yes you are right dream88 .. but can we use IV metronidazole if patient is unable to take it orally ..?

nope Vanco is ineffeective via I/V route because it is poorly absorbed via GI mucosa ( I/V metro preferred in this case).

according to kap, linezolid is the only ORAL med available against MRSA...that means vanco is always IV...

Successor it is not against MRSA, it is vanco use against Clost difficile.that means Vanco...> Oral.

dream88 is right..vancomycin IV does not pass bowel wall so IV vanco is useless in clostridium difficile diarrhea.. when taken orally acts locally on the bowel without gettng absorbed and hence manifesting no sytemic toxicity
