best initial test for perforation cxr or axr

initial is always going to be abdominal xray plain view if a perfered pud is suspected. if confirmed on xray this is usually enough to start w/u for emergent laparotomy. Always order stat on everything that has to do with a pud perf.
Although its true that you can identify with chest xray too, but abdomen covers a larger view point of the upper abdominal quadrants then does a chest xray. For CCS purposes I always do both just to r/o cardiac anomalies...cant hurt Hope this helps.

better to say -- after 10-15 erect position abd -xray
look for free gas under-- dome of diaphragm(not for upeer abd quadrants)

dr90,
It is my understanding that the upper abdominal quadrants begin from the costo-diaphragmatic borders which technically is part of the lower rim of the diaphragm. If not then why do we do U/S abd for gallbladder instead of a chest u/s for gallbladder eval.? Please clarify. |
