is it impossible : fellowship on H1b - drdre2000

previously somebody said it is because univevrsity hospitals only offer or mostly offer J1

drdre2000 is right. most programs do not offer h1 for fellowship.
j1 is the only visa offered. so if u want fellowship option then take j1 visa.

If you are already on J1 during residency and want to do fellowship you can do it on J1, then after that waiver and then GC.

If you are on H1 during residency and want to do fellowship it is difficult although not impossible to find a spot. Keep in mind that H1 is 6 years in duration, can be extended I think only if you applied for GC.
