In 2 year olds, AML is most common. If 2 year old has Downs then AML is still most common, but ALL is seen more often compared to Nondowns pt. Question seems a little oddly worded to me.
If question was asking pure numbers then AML would be the highest risk. However, if question was asking "Which blood disorder is seen more in pts with Downs compared to Nondowns" then ALL would be the answer.
