need to change program - ananida

Hi, I am an IMG and I have already matched last year in a program which It is not as good as I was expected also with a bad outcome in fellowship..
Could I change the program??? (repeat 1st year or 2nd year..) and what is the processing???
n.b. I am on J1-visa.
