whats the reson behind that???becoz of difficulty of getting ER residency or the residency training its self is not good( which i can make sure not be a problem)becoz US and canada are the pioneers of EM in the world

I read ur story and here is my advice for u.
** Its all about ur determination to get into residency. The only thing that u can do now is pass step 3 and start usce in some community hospital with a residency program, so u can make good contacts over there. Stick there for some time and believe me if u would be really determined and excited for ur work, they are gonna take u for their program. Man u have to keep ur moral high and think "yes u can do it".

Dear Dr. Ehmaqa,

One, you are obviously well accomplished so reading your genuine concern and your statement of confusion, is what may be causing you the stress.

First off true EM is VERY COMPETITIVE, and year of graduation does go into the equation as each program has a set cutoff for their applicants.

Having said this this is not a general question, as you know very few countries have EM Boards and if you are part of a system that has this and has completed this, it is a strength for yourself on your application, note your year of graduation maybe 2000 yet you completed your EM board in 2004.. and have been clinically active since then is different than an old grad who was general for 10 years.

Our advice is to pull every single EM residency program in the country each one and make an excel sheet, write the name of the PD, and their min requirements. After which formulate a very professional letter to the program, write this as a co colleague not an applicant, for example

Dear Dr. X,

I'm writing to you as an Board Certified EM Physician from... as you may know my country has implemented EM training in the past ,,,.,, years and we are one of the only countries in the region to have this program. Upon completion of my training, I would like to train in EM in GME in the US, the reason for this, could be for example you want advanced EM training to improve the program in your country ,,, whatever the actual reason explain..

I understand you have a graduation cutoff of X years but will this be waived for a unique case such as mine where I was in an EM training program.

Ask if there is a time you can call and speak with the program directly about your unique concerns, etc..

Many applicants its innapropriate to call the programs or contact them about their concerns yet this is not the case it is all in approach, also call the Board of Emergency Medicine and ask if you could apply for a PGY2 spot or you could qualify to apply for a fellow- ship.

Another option which may or may not appeal to you, is that a large percentage of US EM Physicians do not hold EM boards but FM Boards with a specialization in EM, where an EM fellowship is completed and that becomes their field.

There are many options sit write out a plan for yourself and be very detailed and organized exam scores 1st attempts in the 90s previous board this is a great start!

You have very good chances to get a residency here in US. People are looking for experienced physicians especially university programs. My best bet, ER residency is difficult to get into but that is not the end of this world. With your experience, it should be extremly easy for you to get into family medicine. After 3 years of family medicine, you can easily take an ER route. Its a longer path, but better than wasting many years in search of a competitive residency OR not doing what you have a passion for and rather going for pathology/radiology etc.
