Respiratory (any incision and biopsy) :tonsillectomy, adenoidectomy, or bronchoscopy with biopsy.
GI, GU: AHA says only those procedures with ongoing GI or GU infection. W/o infection on GI and GU procedure is considered high risk and thus prophylaxis is not recommended.
