click and download temporary physician license form ... this is exactly what u will fill when u get residency

ohh okies...but we cannot apply this before getting the Match....one of the forms is to be signed by PD where u got match...I didnt get any prematch offers but that PD told me that he will give me the match...so I have to wait till match then I ll apply for this License...Thanx Shankar and Nita
