Following is the list that is used for scoring rapport and professional manner---there is still 2 more components ---questioning skills and information sharing which I would provide in my subsequent posts....
Rapport and professionalism.
1.knocked at the door
2.greeted , introduced, shook hands warmly
3.informed what he/ she is here for
4.put the drape
5.asked if something from the room , he/ she can help me with
6.Asked permission to write notes or sit down for interviewing.
7.empathized when I told my chief complaint
8.was dressed appropriately
9.was attentive to my concerns other than my chief complaint
10.has ability to encourage the patient to share the issues
11.Greeted well after ending the encounter.
12.Appeared calm and composed.
13.was eager to help
14.Sounded sure of what he wanted to do or say.
15. Made an eye contact.

A lot of such material would be provided during the sessions I take for coaching CS.

yes, I have developed a checklist for all the components and sub components of this exam...I use that to coach my students....
