probably better you direct that question towards your PC, Chief resident or current colleagues.
as my understanding goes, once can only do so after residency.

you can write MD on the labcoat. Depends on the institutional policy. Some places like JHU and affiliated hospitals had MBBS embroidered on the lab coats of IMGs (if MBBS +).

Other places you are free to write MD after the name. followed by the Dept. of ___ on second line. There should be no reservation for most part from the program or hospital to have your name personalized on the labcoat. In fact some places like UT systems will personalize the white coat for you.

i should add that i kept saying lab coat (in error) as a continuation of OPs post. It should be called white coat. UT = university of texas
