Mgm of traumatic haemopneumothorax -

All cases of traumatic pneumothorax require a chest tube high up,to prevent the development of tension pneumothorax. So,a case of traumatic haemopneumothorax should get a chest tube high up to relieve the pneumothorax and a second basal tube to drain the blood.do u agree? If not, please point out the flaw in my reasoning and the alternate answer.

!

A single tube can drain both blood and air.

a tube that is placed high up for a pneumothorax will not drain the blood.Bailey says u should drain the air first and then insert a second basal tube for the blood, while kaplan notes suggest inserting a basal tube only.

big difference between theory and practice. In a patient lying supine a single tube will drain both air and blood. In practice in a trauma situation you place a single chest drain. Where it goes is unimportant. (there is no way of being sure where it will end..what you think is basal often shows in apex on CXR and vice versa). You certainly dont place 2 drains.
