Hi , I am getting a lor from some doctors who asked me write it myself.
He took forever to finally sign the Lor and now I noticed on the letter there is no mention of the hosital and they didn't stamp it saying I didn't waive my right to view it. Is it necessary to have them put that I didn't waive my
Right on the Lor? I'm wondering if I should rewrite it and have him sign but I'm afraid he might take forever.
Any one know what all the LOR must include?
