65yr old woman is to ER b/c lethargy and confusion for 24hr. No fall or injure recently.
Medications include: amitriptyline (150mg bedtime) for major depression, diphenhydramine for the past week (50mg 3 times/day) for pruritic rash. Pulse 100/min and regular, and BP 135/80mmHg.
Physical exam shows a palpable mass in the midline extending 5cm upward from the pubic symphysis. Serum BUN 45 mg/dL, serum creatinine 2.8 mg/dL. What is it?

A)Acute tubular necrosis

B)Bilateral renal artery stenosis

C)Bilateral ureteral obstruction just distal to the renal pelvis

Someone had this question posted several weeks ago and answered as F - because amitriptyline's side effect is urinary retention.
