I am a pgy2 in internal medicine. For family reasons i want to transfer to some hospital closer to my family. Is it hard to find a pgy3 position? anyone have any experience? if i transfer, does it look bad on CV?

It can be difficult to complete this task. You will have to locate an available PGY3 position and be approved for it. In my opinion, it may not necessarily reflect negatively on your CV. You would still have completed the requirement to be board eligible.
