Match dilemma - stress12

This is a stressful period. I hope we all get through this and match somewhere.

I have a question-

If candidate A ranks program X as #1 and candidate B ranks the same program as #4. Who gets the spot?

Scenario1: Program ranks candidate A lowers than candidate B.
Scenario2: Program ranks candidate A higher than candidate B.

I know what would happen if program doesn't rank either of them For now lets assume both of them are ranked below 40 on program's list and the program has 12 spots.

I really appreciate if some one could clarify this for me.

In scenario A if candidate B doesn't get in top 1-3 programs on his or her list then he or she is matched into program A as was ranked higher by program
