respected doctors,my question is that if someone cannot get MSPE from medical school then i heard ECFMG places subsituite letter to complete our application so my question is that if we cannot get mspe from medical school then will it be big red flag in our application?will it decrease our chances of matching in residency program?

really difficult to tell, as none here is actually within the system, to know how programs would value that.
Are you in no way able to get the MSPE? I drafted mine and send it to my old med school.. I understand however life happens, and things change, and your school might be in no way able to provide this for you. you should also try and reach out to the ECFMG and verify with them what the policies are..
best of luck!
