is found on routine chest radiography for life insurance to have right hilar adenopathy. He is otherwise healthy. Besides biopsy of the lymph nodes, which of the following is indicated?
A. Angiotension-converting enzyme (ACE) level B. beta -hCG C. Thyroid stimulating hormone (TSH) D. PSA E. C-reactive protein

I think B. the reason I chose B is, I think it is nonseminomatous tumor, commonly in chest of young adult male, which has increased beta hCG.
The other option is, of course, ACE level, if i suspect sarcoidosis, bit I did not choose that because in sarcoidosis, it will be bilateral lymphadenopathy + there MUST have to have some other extrapulmonary s/s. So, I did not choose that. What is correct answer, cd45 ?

ur reasoning is absoultely correct
ans is B d/d of u/l hilar ln is lymphoma, testicular tumoe esp in this age grp, histoplamosis, tb sarcoid would be b/l ln biopsy would be teh most accurate diagnostic test but bhcg levels and alpha fetoprotein levels should also be determined. |
